Job Description
Full and Part –Time positions, at locations across Sheffield
To work as part of a team to providing person-centred support (which may include personal care), to adults with a
learning disability. To work flexibility to meet the needs of each Client, providing support at a variety of times and
places as needed by the Client e.g. in their home, in the community or at a day centre.
To enable individuals you support to take more control over their lives, develop more skills and increase their
independence by using and developing Risk Assessments (RA), Support Practice Guides (SPG), Person-Centred Plans
(PCP) and Support Strategies (SS) that form part of the Client file.
Opportunity Sheffield is managed by Sheffield City Council and funded by Yorkshire Forward and the European Regional Development Fund.
